credit hold mystery

We have a problem where an inactive BP account is set to active seemingly automatically. When we look at the history it shows that the status was changed by a user who doesn't even have authority to do so. Here is a copy of what the screen shows in history.

Does anybody have any suggestions as to what might be doing this?????
